Know Your Options

The gradual worsening of a chronic illness or a recent hospitalization will leave most of us – at one time or another – with the responsibility of providing personal and/or medical care for a loved one. And, yet, many of us will find ourselves unable to provide the type or amount of care needed. When this happens, there is help. One of the following may be the answer for your family:

Make the Best Choice

Knowing the type of care to select can be difficult. It’s best to talk openly about physical and emotional needs. What type of medical care is needed now and in the future? Is your loved one prone to depression and how can you and others in the support group help meet social needs?

Discuss goals and the best way for those goals to be reached. Is your loved one hoping to recover enough to regain some independence or are you seeking medical care and guidance as a chronic illness progresses?

Definitely consider finances. Do you or your loved one have the financial means to pay out-of-pocket for care or will you need to rely on Medicare, Medicaid, or health insurance?

Once you have a clear picture, you can compare and contrast the features and requirements of different care options.

Take the First Step

A 24-7 Care and Rehab facility may be your best option if:

  1. Your loved one is working towards recovery from an illness or accident and hopes to regain independence OR
  2. Your loved one needs long-term medical care

We invite you to visit and take a tour of the facility closest to you. When you are ready, we will walk you through the admission process. During this process we will:

  • Introduce you to members of the care team
  • Review your payment options and help you determine what insurance, Medicare and Medicaid coverage is available
  • Complete a comprehensive review of your loved one’s medical and clinical records
  • Discuss your loved one’s rights as a resident of a skilled nursing facility
  • Sign paperwork

Develop an Individualized Care Plan

After admission to a 24-7 facility, we will work with you and your loved one to develop a care plan that addresses medical, physical, emotional and social needs and goals. A comprehensive assessment is the first step in developing this plan. In addition to an assessment of the resident’s medical, dental, psychosocial, and cognitive state, we also assess the resident’s:

  • Capability to perform activities of daily living
  • Potential for recreational activities
  • Potential for rehabilitation
  • Potential for discharge

Depending on your loved one’s needs, the care plan may include one or more of the following:

  • Rehabilitation
  • Restorative nursing
  • Diabetic care
  • Complex surgical wounds and pressure ulcers
  • Bariatric care
  • Parenteral medication
  • Ostomy care
  • Tracheostomy and respiratory care
  • Pain management

Get to Know the Care Team

An interdisciplinary group of specialists will be involved in providing the care outlined in your loved one’s care plan. This Care Team includes:

  • Director of Nursing (DON)
  • Director of Activities
  • Speech, Physical, and Occupational Therapists
  • Food Services Director
  • Director of Social Services

Prepare for Discharge

Preparing for the discharge of your loved one starts on day one! Your loved one’s care plan will cover what he needs to accomplish to return home. Generally, your loved one will need to be able to:

  • Bathe, dress and handle other daily personal hygiene
  • Manage medical needs
  • Perform basic housekeeping tasks
  • Navigate the community
